On November 1, 2004, the Personal Health Information Protection Act (PHIPA) became law. PHIPA is Ontario's new health-specific privacy legislation. It governs the manner in which personal health information may be collected, used and disclosed within the health care system. It also confirms a patient's right to access one's own personal health information.
Collection of Personal Health Information
Hôpital Glengarry Memorial Hospital collects personal health information about you directly from you or from the person acting on your behalf. The personal health information collected may include, e.g. your name, date of birth, address, health history, records of your visits to Hôpital Glengarry Memorial Hospital and the care that you received during those visits. Occasionally, personal health information about you is collected from other sources, e.g. from another hospital you have been admitted to, if we have obtained your consent to do so or if the law permits.
Uses and Disclosures of Personal Health Information
We collect, use, and disclose your personal health information:
- to quickly and accurately identify your health record
- to provide effective and appropriate health services and treatment for you
- to get payment for your treatment and care (e.g. from OHIP, WSIB, your private insurer or others)
- to plan, develop and manage our internal operations for delivery of care and services
- to conduct quality improvement activities (e.g. sending patient satisfaction surveys)
You may access and correct your personal health records if deemed necessary. You may withdraw your consent for some of the above uses and disclosures (e.g. patient satisfaction surveys) by contacting us (subject to legal exceptions). Withdrawal of consent is not retroactive and must be provided in writing. Forms to withdraw consent are available from Health Information Services.
Procedures are in place to protect your personal health information from theft, loss and unauthorized access, copying, modification and disposal. We conduct audits and complete investigations to monitor and manage our privacy compliance.

Because HGMH is entrusted with personal health information to treat and care for our patients, we must ensure that we not only educate our staff about PHIPA and the privacy policies and practices implemented, but we must also ensure that privacy becomes embedded into our hospital culture. Each individual affiliated with HGMH has a responsibility to protect patient privacy as we continue to maintain our privacy sensitive culture.
